I'm going to close this topic as I believe the issue is now resolved. The common factor to being locked out were members who had transitioned into the 'registered+' member group. That was because I'd added that new member group *after* configuring automatic chat access. I found the setup where access privileges are specified, and sure enough, I had to explicitly add this new group. Once I did, both registered and registered+ members could access chat.
